Medication Reconciliation Solution from RelayHealth Helps to Address Patient Safety in Emergency Rooms Across the Nation

IntegrateRx - Medication History offers patient’s prescription history to help hospital efficiency and prevent harmful drug interaction

Duane Reade is among those pharmacies that have agreed to make their patients' medication history records available to authorized staff at hospital emergency rooms.

The IntegrateRx medication reconciliation solution allows for seamless delivery of patient data through the hospital's current electronic health record software application. It is designed to replace what is typically a manual - and many times inaccurate - process of interviewing the patient to relate his or her current prescription regimen. Because the solution offers drug interaction screening, it also supports hospitals in meeting the requirements instituted by the Joint Commission.
