Affinity Health System: Using Patient Outreach to Prevent Readmissions
The acute care hospitals in Wisconsin’s Affinity Health System, like other hospitals across the country, are facing a reduction in Medicare payments for preventable readmissions under the healthcare reform legislation. Consequently, there is a strong incentive to reduce readmissions. Affinity set a goal to reduce readmissions of patients diagnosed with heart failure within 30 days of discharge, and launched a heart failure disease management program utilizing RelayCareTM.
