IntegrateRx™—Medication History is a comprehensive solution from RelayHealth which enables hospitals to perform automated medication reconciliation, leading to improved patient safety. Our data comes from customer pharmacies and provides a richer view into a patient's medication history than relying on just Payers/PBM data which is not as robust.
IntegrateRx—Medication History also enables the pre-population of data and vital patient medication information. We enable physicians to populate EMR, ePrescribing or Practice Management Systems with a patient's medication history. This is a one-time service for physicians with a new system which eliminates the need to transfer information from paper charts. The end result is an improvement in efficiency and an increase in patient safety through the reduction of drug interactions.
